Under eye fat repositioning in Korea: what decides the price and the result

How much does under eye fat repositioning cost in korea depends less on the clinic than on which version of the operation you need. Moving the fat pad is the core of it. Whether the surgeon works from inside the eyelid or through the skin, whether fat is grafted into the hollow below, and whether loose skin is removed are three separate decisions, and each one changes the quote.

Our interpreters attend these consultations often, because it is one of the procedures where patients arrive with a photo and leave with a different plan. Bags, dark circles and hollows are three different problems that look the same in a bathroom mirror.

Below is the breakdown. The estimated range is on the Korean eye bag surgery page.

Bag, hollow or shadow

Press gently under your eye and look in a mirror with light from above. A bag is fat pushing forward. A tear trough is a groove below it where the ligament holds the skin down. A shadow can be either of those, or pigment, or thin skin showing the muscle underneath.

  • Fat bulging with a groove below: the classic case for repositioning, where the fat is moved down to fill the groove instead of being removed.
  • Fat bulging with no hollow: removal may be enough.
  • Hollow with little fat: adding volume, by graft or filler, not taking any away.
  • Pigment or thin skin: a dermatology problem, not a surgical one. See pigmentation laser.
  • Loose skin with fine wrinkles: skin excision or resurfacing added to the plan.

Most patients are a mix. That mix is what the surgeon is deciding while examining you, and it is what makes the quote specific to you.

The two approaches, and the laser question

Common approaches in Seoul. Your surgeon chooses based on skin laxity and the amount of fat.
Transconjunctival (inside the lid)Transcutaneous (through the skin)
Where the incision isInside the lower eyelid, no external scarJust under the lash line
Can remove skinNoYes
Usual candidateYounger skin, good elasticityLoose skin or fine wrinkles as well as bags
AnesthesiaLocal with sedation is commonLocal with sedation or general
StitchesOften dissolving, sometimes none visibleRemoved around day five to seven
BruisingUsually lessUsually more

Under eye fat repositioning laser korea listings refer to using a laser rather than a blade to make the incision inside the lid. It can reduce bleeding and some surgeons prefer it. It is a tool choice inside the transconjunctival approach, not a separate operation, and a clinic charging notably more for it should be able to say why.

What is on the quote

  1. Surgeon's fee, which varies with the approach and whether both lower lids are being addressed.
  2. Anesthesia: local with sedation for most cases, general for longer combined plans.
  3. Added procedures: fat grafting, skin excision, a canthal procedure, or laser resurfacing.
  4. Pre-op tests, medication and the eye drops or ointment afterwards.
  5. Follow-up visits while you are in Korea, including the stitch removal if the approach is external.
  6. Anything at a second clinic, such as pigmentation laser for dark circles, which is a separate bill.

If you are comparing two clinics, rebuild both quotes into that list before judging the total, the same exercise as in our itemised quote guide.

Recovery, honestly

Low-salt food prepared for the first days after eyelid surgery (illustrative)
Salt and alcohol are visible in the lower lids faster than anywhere else on the face.

Expect swelling and bruising that peak on day two or three and then drain downward toward the cheek, which is normal and alarming the first time you see it. Sunglasses handle most of it in public from about day five.

Cold compresses for the first 48 hours, head elevated at night, no rubbing, no contact lenses until the clinic says so, and no heat. Most people are presentable at ten to fourteen days, with the final contour settling over two to three months. Our recovery time comparison puts it next to the other procedures.

Reading the before and after photos

Under eye fat repositioning before and after korea galleries are heavily affected by lighting, because the whole complaint is a shadow. A photo taken with light from below removes the bag visually without any surgery at all.

  • Compare photos taken in the same lighting and at the same angle.
  • Check the interval. Three weeks and six months look very different in the lower lid.
  • Under eye fat transfer before and after korea photos show a different procedure: adding volume, not moving it. Do not use one to predict the other.
  • Look for cases with your skin thickness and your amount of loose skin.

If the honest answer for your under-eye is filler rather than surgery, our Ozempic face and pillow face guide covers what under-eye filler does well and where it goes wrong, and injectables gone wrong covers migration.

Choosing the surgeon

Lower eyelid surgery is unforgiving. Taking too much fat creates a hollow that is harder to fix than the bag was, and pulling the lower lid down is a complication that needs a second operation. Ask how many lower lid cases the surgeon does in a month and how they handle a lid that sits low afterwards.

Check board certification and that the clinic is registered to treat foreign patients, as described in our clinic licence guide. If you want a shortlist rather than a search, treatment matching takes about two minutes, or meet the interpreters who cover eye surgery consultations.

  • Ask how the surgeon assesses lower lid support, and what they do if yours is weak.
  • Ask whether the plan touches the skin at all, and what the scar will look like at six months.
  • Ask what happens if a hollow remains after healing, and whether a top-up is included.
  • Mention dry eye, contact lens use, thyroid problems and any previous eye surgery.
  • Ask for the photographs to be taken by the clinic before surgery, in their lighting, so the comparison afterwards is fair.

Questions patients ask us

How much does under eye fat repositioning cost in Korea?
It is quoted per case, and the drivers are the approach, the anesthesia, and whether fat grafting or skin removal is added. The estimated range is shown on our under-eye fat repositioning page, with the final figure set after an examination.
Is the laser approach better?
Using a laser to open the inside of the lid can reduce bleeding, and some surgeons prefer it. It is a tool used within the transconjunctival approach rather than a different operation, so judge the surgeon and the plan first.
Will there be a visible scar?
With the transconjunctival approach the incision is inside the lower lid and leaves no external scar. With the external approach the incision sits just under the lash line and usually fades, though healing varies by person.
How long is the recovery?
Bruising and swelling peak in the first days and most people look presentable after ten to fourteen days. The contour keeps settling for two to three months, so early photographs are not the final result.
Can the bags come back?
Repositioned fat generally stays where the surgeon put it, but the face keeps ageing and skin laxity continues. Some patients need skin tightening or resurfacing years later rather than a repeat of the same operation.
Should I have filler instead?
Filler can camouflage a tear trough in the right candidate and is reversible with hyaluronidase, but it does not remove a bulging fat pad and it migrates in some people. Your doctor decides which applies after examining you.
