Thick calves in Korea: botox, nerve block or actual surgery

Calf reduction surgery is one of the requests that brings people to Seoul specifically, because the operation is uncommon elsewhere and openly advertised here. It is also one where the word covers several very different things, and the differences matter more than usual.
Before any of them, one question decides everything: is the bulk muscle or fat? Squeeze the calf while standing on tiptoe. Muscle hardens. Fat does not. A surgeon confirms it properly, and often with imaging, but that ten-second test at home tells you which conversation you are about to have.
Muscle and fat have opposite answers. Treating one as the other is how people spend money on a result they cannot see.
The three options, honestly described

The two middle rows are where caution belongs. Reducing the function of a muscle you walk, run and climb stairs with is not a cosmetic decision the way an injection is, and the trade-off should be explained to you in detail before you consent. Calf botox: what it is realistically good for
Toxin injected into the calf muscle reduces its activity, and over weeks the muscle becomes less prominent. The change is modest, it builds over about a month, and it fades. Most people who like the result repeat it a few times a year.
The muscle involved is usually the gastrocnemius, the one that bulges when you rise onto your toes. What it will not touch is the deeper muscle underneath or the bone beneath both, which is why two people with identical-looking calves can get very different results from the same injection.
- It suits genuinely muscular calves in someone who is not relying on those muscles for sport.
- The change is measured in centimetres at best, and photographs flatter it more than mirrors do.
- Walking can feel different for a while, particularly on stairs and in heels. Say so if it does.
- Dose and placement matter more than brand, which is the same story as everywhere else on the face.
- If you train seriously, discuss it with the doctor. Weakening a calf is not free.

If you are considering the surgical route
Muscle resection is real surgery on a limb you need every day. Expect a scar behind the knee or in the crease, compression garments, restricted walking for a period set by your surgeon, and a slow return to exercise.
- Ask how many of these the surgeon performs, not how many the clinic performs.
- Ask about calf strength afterwards, in specific terms: stairs, running, heels, standing all day.
- Ask where the scar sits and how it behaves in someone with your skin, because this area scars visibly.
- Ask about blood clot risk and what happens on a long flight home, which is a real consideration after leg surgery.
- Ask for the follow-up schedule and whether it is inside the price.
Do not book a flight for a few days after leg surgery. Discuss the timing with your surgeon, plan a longer stay than you think you need, and arrange somewhere you can keep the legs elevated. The honest version of the outcome
Calf shape is set by muscle, fat and bone. Surgery and injections change the first two. Nobody changes the third, and the length of your lower leg is fixed. That is why the most satisfied patients are the ones who arrived wanting a specific, modest change rather than different legs.
A last practical note for anyone comparing clinics from abroad: this is a treatment with a strong before-and-after culture and very little standardisation in how those photographs are taken. Ask instead how many units or how much muscle, and how the surgeon decided. A clinic that answers in numbers is easier to compare than one that answers in galleries.
